Registration
If you wish to join the practice and live in the LE3 area, please come in and collect a registration form. We will require an official form of photo ID and proof of address for all patients that wish to join us. For a baby we only require the birth certificate.

You can order Repeat Prescriptions by Email currently, please see the repeat prescription section of our website for details.
Online services are not yet available. We hope to provide this service in the future.
